    Of course there would be difficult situations. As it is now, many people are bankrupted with medical situations. If a patient is aware of the costs in advance, they may choose to go to the public community hospital over the gold-plated research hospital. They may decide some of the tests are unnessasary. They may decide to spend less time recovering at the hospital and return home sooner.
    Hospitals, churches, and community groups could have pools setup to help the poor with their copays. But the patient would still be informed of the costs, and be responsible for petitioning the charity for assistance.
    Of course my plan is completly theoretical, because there is zero chance of getting liberals to support a plan that causes everyone to pay something out of pocket.

    My health plan is to have all people responsible for a minimum 10% co-pay on all medical costs. This includes medicare, government workers, union workers, medicaid recipients.
    Before starting any operation or treatment, the doctor or medical center must provide an estimate of the procedure's costs, the same as an auto mechanic. In most cases, the medical center can't bill the patient or insurance for more than the initial estimate.
    If the patient thinks the cost is too high, they can choose another provider or ask for some of the items be skipped. If a patient has limited days to live, they may decide a $100,000 experimental treatment is not so needed if they are going to have to pay $10,000 of the cost out of pocket.
