I believe that WaPo et al. (NYT, CNN, MSNBC, etc.) are reporting real leaks that they get. The are interpreting things through a filter based on their negative view of Trump. However, they are not making up leaks to manufacture news. Furthermore, the bias of WaPo et al. simply reflects what the center and left think about Trump. Many Trump supporters are claiming day in day out that there is a media conspiracy that is making all of these people think that Trump is an idiot. They have it backwards. The so called fake news just reflects what the other half of the country thinks is true. Trump exacerbated a split in this country with his campaign, and his war on the MSM is putting the nails in the coffin. The partisan divide in how the country views the press is exploding (I put a chart of it in another thread).
