Coronavirus disease 2019 (COVID-19) messenger RNA (mRNA) vaccines have been associated with numerous side effects since their widespread release to the public. Cardiovascular complications include myocarditis and pericarditis, Takotsubo cardiomyopathy, postural orthostatic tachycardia syndrome (POTS), arrhythmias, sudden cardiac death, and cardiac tamponade. Pulmonary complications are pulmonary embolism (PE), interstitial lung disease (ILD), idiopathic pulmonary fibrosis (IPF), pneumonia, eosinophilic granulomatosis with polyangiitis, pneumonitis, and pulmonary hypertension. Despite these complications, the risk-benefit analysis still strongly favors vaccination, as these events occur more frequently with natural infection and confer a significantly worse prognosis. This study outlines the evidence surrounding each attributed effect, the clinical course including diagnosis and management, and the proposed pathophysiology. To our knowledge, this is the most comprehensive review of the cardiopulmonary effects of COVID-19 vaccination to date.

Background Myocarditis is a rare but potentially serious complication of COVID-19 vaccination. Cardiac magnetic resonance (CMR) with late gadolinium enhancement (LGE) imaging can identify cardiac scar, which may improve diagnostic accuracy and prognostication. We sought to define the incidence of long-term LGE post COVID-19 vaccine-associated myocarditis (C-VAM) and to establish the additive role of CMR in the diagnostic workup of this condition.

Methods Patients with Brighton Collaboration Criteria Level 1 (definite) or Level 2 (probable) C-VAM were prospectively recruited from the Surveillance of Adverse Events Following Vaccination In the Community database to undergo CMR at least 6 months after diagnosis. As there were limited patients with access to baseline CMR, prior CMR results were not included in the initial case definition. The presence of LGE at follow-up CMR was then integrated into the diagnostic algorithm, and the reclassification rate (definite vs probable) was calculated.

Results 67 patients with C-VAM (mean age 30±13 years, 72% male) underwent CMR evaluation. The median time from vaccination to CMR was 548 (range 398–603) days. 20 patients (30%) had LGE. At diagnosis, nine patients (13%) were classified as definite and 58 (87%) as probable myocarditis. With the integration of CMR-LGE data, 16 patients (28%) were reclassified from probable to definite myocarditis.

Conclusion LGE on CMR occurred in one-third of patients with C-VAM. Without CMR at the time of diagnosis, almost one-third of patients are misclassified as probable rather than definite myocarditis, indicating a diagnostic strategy using echocardiography alone is insufficient.

Synthetic mRNA Vaccines and Transcriptomic Dysregulation: Evidence from New-Onset Adverse Events and Cancers Post-Vaccination

This study provides transcriptomic evidence of molecular disruptions in two patient populations—those with new-onset nonmalignant adverse events and those with newly diagnosed cancers—months to years following COVID-19 mRNA vaccination. Using differential expression analysis and GSEA, we identified hallmark signatures of mitochondrial dysfunction, translational stress, immune dysregulation, endothelial disturbance, and proliferative signaling across both cohorts. Notably, while both groups shared transcriptional perturbations in immune and translational pathways, the cancer group exhibited additional signatures of genomic instability and epigenetic remodeling.

Persistent spike protein expression, prolonged synthetic mRNA activity, and RNA modifications such as N1-methylpseudouridine appear to contribute to sustained aberrant ribosomal activity, proteostasis stress, and immune activation. Our findings also highlight transcriptional signals indicative of tumor-promoting conditions, including suppressed p53 networks, activated MYC targets, and altered interferon signaling, particularly in the context of epigenetic dysregulation in the cancer cohort. These observations suggest that vaccine-induced transcriptomic reprogramming may differentially affect individuals, genetically or immunologically over a long period of time after vaccination.

The coronavirus disease of 2019 (COVID-19) has the potential to cause or promote a number of health problems in both the short and long terms. It is often claimed that the COVID-19 messenger RNA (mRNA) vaccinations can reduce the severity of COVID-19 and its sequelae. We counter this claim with evidence and moreover hypothesize that the mRNA vaccinations are triggering mechanisms that result in an amplification of COVID-19 morbidity along with the “post-acute sequelae of COVID-19” or PASC. Since 2021, the coronavirus infections were often superimposed on a preexisting mRNA vaccine-induced milieu of toxic spike protein, inflammatory lipid nanoparticles, and residual process-related DNA impurities. Spike proteins resulting from both the mRNA vaccination and the natural SARS-CoV-2 infection have been shown to persist for extended periods, raising parallel concerns regarding potential implications for long-term safety with this vaccination. Many, if not most, of the morbidity and mortality events attributed to COVID-19 in extensively-vaccinated populations in 2022-2023 were due to the long-term background persistence of spike protein and other vaccine-associated components resulting from previous COVID-19 mRNA vaccinations. Although close temporal associations with laboratory-confirmed SARS-CoV-2 often engendered the assumption of viral causation, the morbidity and mortality events are plausibly attributed to a combination of mass vaccination and coronavirus infections. The overlapping spike-related toxicities and immunological effects of mRNA vaccinations and coronavirus infections have resulted in pronounced immune dysregulation and inflammatory cascades that likely account for near-synchronous waves of COVID-19 and all-cause mortality. We present research suggesting a potential for coronavirus infection-induced amplification of adverse events (AEs) associated with the COVID-19 mRNA vaccinations, including a diverse range of cardiovascular, hematological, autoimmune, and neurological conditions that frequently overlap with PASC. In this paper, we coin the phrase “Hybrid Harms Hypothesis”, which calls for re-examining the unique immunopathological dynamics of coronavirus infection in COVID-19 mRNA-vaccinated individuals and their implications for public health strategies.
