I believe that it's a question of thickness and hitting a lot of areas at once. If a guy can hit the clitoris, the g-spot, and in/around the cervix region, while giving the woman a 'stretch', then it's likely that she'll have an orgasm.

I'd only met one gal like that in my life. And yes, she was a girlfriend for six months until she'd moved away. She was extremely tight (& I'm not thin); I'd gone in from behind, with her lying over my body, but with both of us facing up towards the ceiling. It took a lot of lube (she's dry at the start) and moving slowly to warm her up. And unlike those who were always faking it with moans and groans, she was silent until her body starting rattling. The way I looked at it, all jokes aside, is that many women are fakers, which is just another reason why I'm sort of turned off by relationships. I mean why not just hire someone for the hour or two than in having a relationship where she's faking it, probably half the time.
