If there's the outline of a half sphere above the nipple then they are always fake. Look at the tits in the next image bellow the one you commented on.
There's no circular outline. Those are real, the ones you commented on are not.
Also no matter how big they are, there should always be a little sag at the bottom. If they just jet straight out from the bottom with very little droop, they are fake as hell.
