The first step is to stop allowing citizens of known terror states or places with active Fundamentalist Jihadi Rebellions from entering the country at all. Require visitors to declare ALL their nationalities and present passports for any State they are resident or citizen of. If they refuse, they are refused entry. If they lie, they get arrested and sent back. Or better yet, arrested, punished,and then sent back, even if it costs a few bucks, to broadcast a few examples.
The second step is to impose harsh vetting and require non-American visitors to unlock their smartphones and leave them in custody of Federal Investigators during their stay.
The third step is to deport those known to be Jihadi Supporters who are here with Green Cards or Visas. Let the cheapass $300k/year admins and execs howl that they can't get cheap foreign labor!

You are the president of the USA. What do you do?

I ask the CDC and intelligence agencies what type of biological agent it is and what we believe the intended transmission method is. At a kill of 10 million, it is likely a rather significant volume of material. It will need to come in on the attacker or via some other concealed method.

I'd immediately look to try and restrict/find the weapons/material itself and prevent it from entering the country. If the only potential there is to leave all possessions at the door, for refugees entering, while we take a month to examine all possessions, than that's what we would do. Piles of clothes, bags, everything ... to be searched, cleared, and mailed back to owners.

Crucially important : we never tip our hand or do any reaction that scares these guys off. I want everything to seem normal and good. No alarms. I want that material to walk right into my control. It's far more important to find the biological agent, and get control of it, than actually get the person themselves.

At 10 million deaths we are likely talking some formerly state controlled actual biological weapon, now in the control of someone very bad. If true, my money would be on former soviet biologic programs, and seriously, we must be talking the absolute worst case weapon: something like weaponized small pox.

The person themselves likely leads to the end goal (control of the material), so I ask intelligence Russians/Isralies/US what known associates, factions, people are known to have given the weapon to the individual, who trained them, anything and everything we know about the circles they move in. There should be a full profile workup on the would be attacker(s).

We would need to also spin up a specialized task force which becomes every moment of their waking days purpose: find and capture weapon. Additionally, need to quietly get FEMA prepared for if we miss. All the better if we know "what" this stuff is, but we better be ready if they succeed.

If you think closing down refugee entry into the US stops this alone, you would be sadly mistaken. They could do something as simple as internationally mail the material/weapon into the country for someone else as well. Where there is a single capability like this, there will be another. So even if this attack cannot be thwarted, you need to continue to trace where this came from, the likelihood of more, and on and on.

Those that said, "suspend all refugees from those countries" gave a simplistic answer to likely a very motivated entity who will find another easy means of entry. Also, you realize these people are vetted for literally years. It has to be one of the HARDEST ways to get into the US: a refugee. These attackers aren't going to come diplomatically or legally via a refugee program. IF they re that stupid, netting up their possessions should get the weapon.

Far more likely though, that any actual attacker, would be A) already a citizen and B) the weapon was made domestically.
