Ian Welsh is an idiot. Obama ended the Bush tax cuts, removed troops from Iraq, tried to close Guantanamo Bay, tried to fix health care, et. al. He should have done more, especially w.r.t. civil liberties, but he wasn't Bush's third term.

Expanded affordable coverage for the poor (causing prevention related efficiencies) and coverage for those with preexisting conditions are the two biggest positives of the ACA. Way less people end up waiting until they are in critical condition and showing up at the emergency room with advanced disease.

Secondary is kids being on parents plans until what ? 25, or 26.

Biggest negatives are that insurance companies and the profit motive are still too much involved in channeling what people pay for health care towards providers. Obama correctly calculated that it wasn't yet the time to go for a more complete solution.

The obvious forces acting as obstacles to reform are those who don't believe that the poor should get the same quality of health care as the rich. I'm not even weighing in on that - but it's so obviously the big philosophical obstacle. (In this case rich would include those working people with quality health insurance).

Big questions exist: what quality of health care could reasonably be delivered to everyone ? And what changes have to occur in our health care and possibly even economic models before everyone could get good high quality health care ? Who pays for it, and how ? Does everyone deserve it ? We are told everyone deserves to be born.

Who pays for the quality health care provided to those who are disabled, those who don't work or can't work ?

There are a lot of reasons why this can't shift in a huge way all at once. It's not Obama's fault. He took as much of a step as he thought he could. This is how progress occurs, one step at a time.

It's easy enough to not take Nexium or Januvia and use OTC stuff from WalMart instead. I even get my cat's insulin (Humilin N) at the WalMart pharmacy without a prescription, and it's only $25 and unlike the stuff made specifically for cats (ProZinc, which is $130) the Humilin N doesn't lose it's effectiveness so quickly after first using the bottle.
